Our Location and Contact Information:
Currently,
we do not have a physical office space accessible by clients or
consumers, however, we do travel to the location of the client or
consumer, so they do not have to travel. Once we do have our
publicly accessible office space in place, we will welcome consumers
and clients to that location.
However, we will alwaysbe willing and availble to travel to a client's location, if requested.

Within the next 4 years, grow Connecticut Behavioral Outreach, Inc.
into a competitive Non-Profit services provider that positively impacts
the mentally ill people of Connecticut between the ages of 17 and 65
that specializes in outreach and advocacy for the target population,
ensuring rapid, effective, and appropriate transitions to and from
various residential settings with coordination of benefits required to
support those settings.
CTBO, Inc. formed to create avenues for strengthening the processes and
resources for CT's mental health communities of care and related
systems. We will provide effective and efficient advocacy for mentally
ill individuals so that they do not have to self-advocate. We will
ensure proper and timely inception of appropriate government-funded
benefits, as well as rapid and appropriate placement of individuals
into needs-based residential solutions. CTBO, Inc. will have it's own
residential facility, as well as a vigorous day-care program for
mentally-ill individuals between the ages of 17 and 65. We will provide
transitional services and outreach to ensure smooth transitions from
the adolescent mental health systems to the adult mental health
systems, and from the adult mental health systems to the geriatric
mental health systems.
Our Mission:
The mission of Connecticut Behavioral Outreach, Inc., is to provide
outreach, advocacy, and coordination of benefits for individuals who
have a serious mental illness. To date, CTBO has identified a profound
gap in the services provided by current communities of care. Although
The Social Security Administration, Department of Social Services, CT
Homeless Shelters, and Behavioral Health Facilities provide many
resources to the mentally ill, CTBO will close the gap on providing
proactive outreach and advocacy, because mental illness often prohibits
cognition that does not allow the individual to advocate for themselves
in this capacity.
In addition, due to chronic understaffing and underfunding of current
communities of care to be able to perform this type of advocacy when
needed, CTBO would fill that gap as well, allowing those agencies to
concentrate more fully on addressing the day to day and emergency
mental health needs of the client.
CTBO will serve a specific population based on age of the client. The
age range will be from 17 (to proactively prepare the client for adult
issues and requirements), to the age of 65 (when clients will be
preparing to transition into mental health services for the elderly). Our Objectives:
1.
CTBO will move 20% of homeless mentally ill of the target population
into appropriate housing with benefits within 4 months of initial
contact.
2.
CTBO will engage mentally ill individuals in initial outreach and
advocacy dialogue to offer CTBO's services with 50% of the target
population in CT.
3. CTBO will create day-care services for the target population 12 months after first client is served by CTBO.
4. CTBO will create supportive housing for the target population 12 months after initial client is served by CTBO.
5. CTBO will reduce by 20% the homeless population of the target population each year of operation.
6. CTBO will provide outreach and advocacy services to 40% of the target population each year of operation.
7. CTBO will attain a satisfaction rating among served clients of 95% each year of operation.

CTBO will partner with all government, nfp, and private sector entities
to enhance and uplift potential and current clients of the target
population and their family members on a continual basis each year of
operation. Our Strategies:
1. Provide Outreach to all mentally ill persons in the target population
2. Provide Advocacy to all mentally ill persons in the target population
3. Provide Day Care services to all mentally ill persons in the target population
4. Provide Supportive Housing to all mentally ill persons in the target population
5. Provide training for volunteer staff
6. Provide Educational Outreach for the State of CT
7. Provide Educational Outreach to local entities
8. Provide Educational Outreach to the Public at large
9. Provide a Warm line
10. Provide a Hot line
11. Engage community members for volunteer services Our Goals:
1. Recruit and retain talented, committed staff to achieve mission directives by 10/30/08.
2.
Write grants to support each business facet, from staffing and benefits
to providing outreach and advocacy services with follow up.
3. Acquire 3 million in funding by 12/31/08.
4. Maintain business objectives for year 2008 and beyond.
5. Acquire additional 3 million in funding for each of the next four years.
6. Create and implement self-sustainability model by 12/31/09.
7. Utilize all "green" technology where appropriate on a continual basis.
8. Have telework plan in place and implemented by 12/31/08.
Our immediate needs:
1. Volunteers with expertise in the areas of writing grants.
2. Financial donations (Paypal link will be up shortly).
3. You to "spread the word" to individuals whom we could serve.
4.
Board members in the areas of psychiatry, non-profit legal, non-profit
accounting, mental health consumer between the ages of 17 and 65,
mental health advocate, Social Security, Department of Social
Services in Connecticut, and any other individuals who feel passionate
about changing the world that mentally ill people exist in currently in
the State of Connecticut. Our long-term needs:
1. Volunteers to manage a "warm" line.
2. Volunteers to manage a "hot" line.
3. Volunteers to mentor mentally ill young adults.
4. Financial donations.
5.
Anyone else who feels they would like to participate in a voluntary
capacity in our mission to reinvigorate the lives of the mentally ill
in Connecticut.

1. Volunteer for one of the positions above (or think up a different volunteer role for yourself!)
2. Send monetary donations.
3. Share your expertise as a non-profit business mentor to our newly forming organization.
4.
Sponsor the needs of a particular part of our organization, so that we
can effectively serve the disenfranchised population of mentally ill
individuals in Connecticut.
